Red flags — what to avoid when choosing a foreign medical university

These are patterns that consistently lead to NMC registration problems, FMGE failure, or wasted years. Know them before you commit.

Newly set-up colleges (less than 10 years old)

High risk

Institutions established recently carry a higher risk of sudden derecognition, infrastructure shortfalls, and uncertain accreditation status. Less established colleges may not have sufficient clinical training hospitals or faculty. If a college was set up after 2015, investigate its accreditation history very carefully.

Non-English medium programmes

High risk

FMGL 2021 requires the MBBS programme to be in English throughout. Many programmes in Russia, China, and Central Asia are conducted partially or entirely in the local language — especially clinical years. If a programme is not English-medium throughout, it does not meet NMC requirements. Always ask for written confirmation of the medium of instruction.

No affiliated hospital / insufficient clinical exposure

High risk

A medical college must have an adequately staffed teaching hospital. Many foreign institutions arrange clinical rotations in under-equipped settings, which leads to poor FMGE preparation. Ask for the patient load, bed count, and specialty availability at the attached teaching hospital.

Transfer restriction violations

High risk

FMGL 2021 (effective 18 Nov 2021) prohibits mid-course transfer to a different university or country. Any agent or institution that offers a 'transfer pathway' or 'lateral entry' to a better-ranked institution mid-course is describing something that violates NMC regulations (with the narrow exception for Ukraine-returned students under the 2022 circular).

Agents promising 'guaranteed NMC recognition'

Critical

NMC does not publish a list of recognised foreign institutions and does not 'recognise' individual universities in advance. Any agent claiming a specific university is 'NMC recognised' or 'approved by NMC' is making a misleading claim. NMC recognition is student-specific and assessed at the time of FMGE/NExT application based on compliance with regulations at the time of the student's enrolment.

Institution not listed in WDOMS

Critical

The World Directory of Medical Schools (WDOMS) is the global reference for recognised medical schools. NMC uses WDOMS listing as a prerequisite. If an institution is not listed at wdoms.org — or was listed after your enrolment but not at the time — it may not satisfy NMC requirements. Verify the WDOMS listing personally, not through the agent.

Always verify WDOMS listing yourself

Do not rely on an agent's word for WDOMS listing. Go to wdoms.org and search for the institution directly. Verify that it was listed at the time you plan to (or did) enrol.
